PLEASE READ THE FOLLOWING IN FULL BEFORE SIGNING
In consideration of participant as listed in this form my child/ward if under 18 or myself if 18 and older, being allowed to participate in Jefferson Theatre Company’s summer musical and related activities, the undersigned acknowledges, appreciates, and agrees that:
Participation in any program that has physical activity exposes the participant to certain risks and dangers. Accidents and injuries are always a possibility and it is impossible to foresee and protect a participant from all conceivable danger ; and,
1. FOR MYSELF or on behalf of my minor child, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, even arising from the negligence of others, and assume full responsibility for my or my child’s participation; and;
2. FOR MYSELF, or on behalf of my minor child, I HEREBY RELEASE AND HOLD HARMLESS Jefferson Theatre Company and Jefferson Theatre Company Booster Club, its directors, officers, agents, employees, volunteers, other participants and if applicable, owners and lessors of premises used to conduct the event (“Releasees:), WITH RESPECT TO ANY AND ALL INJURY, or loss or damage to person or property incident to my or my child’s involvement or participation in these programs.
3. FOR MYSELF, or on behalf of my minor child, I HEREBY INDEMNIFY AND HOLD HARMLESS all the above Releasees from any and all liabilities to my involvement or participation in these programs.
4. I, as participant or the parent/guardian, assert I understand this agreement.
I, FOR MYSELF AND CHILD/WARD, HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT WE HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
